Employee Health Requirements – Communicable Disease Prevention

1. Early Head Start (EHS) staff (and consultants with frequent EHS involvement) are required to show proof of:
   1. PPD testing and/or follow-up
   2. MMR vaccination
   3. Varicella vaccination (or documentation of clinical illness)
   4. Influenza vaccine (Including H1N1)
   5. Pertussis vaccination
   6. Annual Employee Health Appraisal
2. HS staff in High Risk Settings (First Place, Willamette Family Treatment) are required to show proof of:
   1. PPD testing and/or follow-up
   2. Annual Employee Health Appraisal
3. All other HS Employees:
   1. Annual Employee Health Appraisal
   2. Annual Tuberculosis Screening Questionnaire
